Introduction

Welcome to USeeBP, a free blood pressure management mobile application ("App") provided by Qualia Health, Inc. ("we", "our", or "us"). This App is intended for use by patients at the University of Chicago Medicine ("UCM," "we," "our," or "us") and its affiliated medical practices who are enrolled in the mHealth for Hypertension Study (University of Chicago IRB23-1110). By installing and using this App, you ("you," "your," or "User") agree to abide by these Terms of Service ("Terms").

Acceptance of Terms

By accessing or using the USeeBP App, you acknowledge that you have read, understood, and agreed to be bound by these Terms. If you do not agree, you may not use the App.

Eligibility

To use the USeeBP App, you must be a patient managed by UCM or its affiliated medical practices and must be enrolled in the mHealth for Hypertension Study (University of Chicago IRB23-1110). You must also be at least 18 years of age.

Medical Disclaimer

The App is designed to assist you in managing your blood pressure and is not intended to replace professional medical advice, diagnosis, or treatment. Always consult with your healthcare provider for medical advice and treatment.

Privacy Policy

All research performed through the University of Chicago is governed by their Privacy Policy which can be found here.

In addition, the USeeBP application has a more specific privacy policy which can be found on this website.

Use of App

The App is provided free of charge. You agree to use the App responsibly and in accordance with all applicable laws. You may not reverse-engineer, decompile, or disassemble the App or attempt to gain unauthorized access to the App or its systems.

Data Accuracy

While we strive to provide accurate and up-to-date information, we make no warranties or guarantees regarding the accuracy, reliability, or completeness of the data provided by the App.

Limitation of Liability

To the fullest extent permitted by law, UCM shall not be liable for any indirect, incidental, or consequential damages resulting from the use of the App.

Termination

UCM reserves the right to terminate or suspend your access to the App at any time, without notice, for conduct that violates these Terms or is harmful to UCM, its affiliates, or other users.

Changes to Terms

We reserve the right to modify these Terms at any time. Your continued use of the App after any changes signifies your acceptance of the updated Terms.

Governing Law

These Terms are governed by the laws of the State of Illinois, without regard to its conflict of laws principles.
